Billing and Insurance

Patients are responsible to pay any deductibles, percentages due or co-pays at the time of visit. We accept cash, check, VISA, Mastercard, American Express and Discover.

Our Financial Coordinator will assist you with your financial questions including fees, insurance coverage and payment options. Patients are responsible for providing information regarding their insurance coverage to our staff. As a courtesy to our patients, our office will file your visits with your health insurance company. This is a service that we provide, but please remember that the contract is an agreement between you and your insurance company and you are ultimately responsible for payment. We cannot accept responsibility for collecting payment from your insurance company nor negotiating a settlement on a disputed claim.

Many insurance companies do not cover certain procedures or treatments common with infertility care. Patients should meet with our Financial Coordinator to obtain a predetermination of coverage letter from their insurance provider prior to beginning any infertility treatments or in vitro fertilization. This letter will outline deductibles to be met, co-insurance responsibilities, plan limitations, and procedures not covered. Although we will gladly assist patients in obtaining information regarding insurance coverage, it is the patient's ultimate responsibility to pay for all services not covered by their plan. If we do not receive payment within 60 days from the insurance company, it is the patient's responsibility to pay FWHCLA the balance owed and settle with their insurance company.

The ARC Family Building Program^TM

Managing your infertility treatment means that you may to have make some difficult decisions about your medical options, including financial ones. Through ARC you can secure reasonable financing for many of your medical services covering many aspects of your infertility care.
